Registration Requirements
Documents required for registration. If any documents are missing the appointment will need to be rescheduled. Having all of your documentation will help expedite the enrollment process.
- Proof of residency (Two of the following)
- Rental or lease agreement signed by the property owner or designee and lessee listing residence location and date of occupancy and names of all individuals living in residence
- Settlement statement listing the residence location and date of occupancy
- Deed listing residence location
- Current utility bill
- Current Credit Card Bill
- Property of Tax Bill
- Department of Transportation or Valid ID Card
- Vehicle Registration
- Proof of Child’s Age (One of the following)
- Original state-issued birth certificate
- Baptismal certificate
- Copy of the record of baptism
- Notarized or duly certified statement and showing the date of birth
- Notarized statement from the parents or another relative indicating the date of birth
- A valid passport or a prior school record indicating the date of birth
- Copy of current immunization record
- Student may not, by law, start school until their immunization record is received by the District
- If student was born outside United States or recently traveled outside The United States, evidence of TB test and results are required
- Custody orders, if applicable
- Name of previous school with address, phone number and fax number
- Special Education Documentation (IEP, NOREP, ER, RR), if applicable
- Other educational plans (GIEP, 504 agreement), if applicable
- Other documents that will be helpful
- Report cards
- Transcripts
